Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) On or about July 12, 2016, during roofing work, at a site located at 2656 S. Oneida Street, Green Bay, WI 54304, the employer failed to ensure that employees used a fall protection system when exposed to falls of approximately 24 feet to the ground floor when working on a parapet of a steel building on a low slope roof.
