29 CFR 1926.1408(a)(2): Determine if any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line. If so, the employer must meet the requirements in Option (1), Option (2), or Option (3) of this section.    a.  At the worksite located at 2375 Stringtown Road, Grove City, Ohio, the employer did not ensure the required protections pursuant to 1926.1408(a)(2)(i) [Option 1], 1926.1408(a)(2)(ii) [Option 2], or 1926.1408(a)(2)(iii) [Option 3] were met when it was determined the hoisted load could get closer than 20 feet to the adjacent power lines.  On or about July 10, 2016, a billboard panel that was being hoisted came into contact with 7,620 Volt power lines, resulting in serious electric shock injuries to an employee.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.       a. The employer did not report to OSHA an in-patient hospitalization. An employee was injured on or about July 10, 2016 that required in-patient hospitalization.
