OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to hazards created by ingoing nip points:    At the workplace, on the output side of inspection table #5, employees were not protected from the ingoing nip point of the manufactured conveyor belt and floor mounted guide roll while performing the task of pulling trim.  On or about June 23, 2016, an employee was pulling trim on the output side of inspection table #5 with the machine in jog mode and light curtains bypassed when the trim broke, allowing the employee to fall onto the manufactured conveyor belt that was traveling under a guide roll, thus pulling the left arm into the unguarded ingoing nip point, resulting in crushing injuries to the arm.    Among other methods, on feasible and acceptable abatement method to correct this hazard would be to ensure all necessary guarding is in place and operating properly to prevent employee exposure to the hazards created by ingoing nip points.
