29 CFR 1926.153(g): Containers and regulating equipment installed outside of buildings or structures. Containers shall be upright upon firm foundations or otherwise firmly secured. The possible effect on the outlet piping of settling shall be guarded against by a flexible connection or special fitting.    a.  At the front of the building where the employer had a tar kettle set up and heating the tar, the Project Superintendent laid 4-100 gallon cylinders of liquid petroleum gas in the bed of the truck and transported the cylinders to the jobsite thereby exposing employees to a fire/explosion hazard.

29 CFR 1926.501(b)(3): "Hoist areas." Each employee in a hoist area shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems. If guardrail systems, [or chain, gate, or guardrail] or portions thereof, are removed to facilitate the hoisting operation (e.g., during landing of materials), and an employee must lean through the access opening or out over the edge of the access opening (to receive or guide equipment and materials, for example), that employee shall be protected from fall hazards by a personal fall arrest system.    a.  On the upper section of the roof where employees were observed hoisting boards up to the roof on the forks of a Gradall Rough Terrain forklift and the employer did not provide a conventional fall protection program thereby exposing employees to a fall hazard.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines.    a.  On the second level where employees accessed the roof to walk to a ladder to gain access to the upper roof, the employer did not install control lines from the access door to the work area thereby exposing employees to a fall hazard.
