29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Front of site near road: An employee was working in an un-protected excavation approximately five (5) feet deep in loose sandy soil where a ramp or ladder had not been provided for safe access and egress exposing the employee to cave-in hazards. (b) Rear of site: Employees were working in the foundation walls approximately ten (10) feet below grade where no method of access or egress was provided. Employees must exit the protection of the foundation walls and scale the unprotected excavation to the surface grade exposing the employees to cave-in hazards.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: (a) Rear of site: The face of an excavation was not adequately guarded to prevent loose soil and rocks from falling into the excavation. This exposed the employees to struck-by hazards where the employees had to travel through an unprotected excavation that was approximately ten (10) feet deep in loose sandy soil and climb the end to exit.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). (a) An employee was working near the roadway at the front of the site in an unprotected excavation approximately five (5) feet deep with loose sandy soil. The employee was bending over with a hand shovel clearing soil from an area in an attempt to locate the water main. The employee was exposed to cave-in hazards.
