29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:     (a) On or about July 14, 2016, at the facility located at 3950 N Kimball Drive - Kansas City, MO., employees were exposed to serious eye injuries, in that employees wearing prescription lenses  were not protected from eye hazards in areas or operations where eye protection is being required. Prescription eye glasses must meet ANSI Z87.1  requirements and be used with side shields or be used with other eye protection wore over them.       Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
