29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved:     a) 7000 Millington Road, Clayton, DE - On or about July 06, 2016, the employer failed to instruct every employee in the safe operation and servicing of all harvesting equipment with which he was involved.  An employee, who had not received the instruction, was using a pressure washer and hose to clean a Pea Harvester when the hose became entangled in the Spinner on the front of the machine, pulling the employee's leg into the spinner, resulting in multiple leg fractures.       Abatement Note:    At a minimum, the employer shall instruct each employee as outlined in paragraph (a)(6)(i) through (a)(6)(v) initially and annually thereafter.
