29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. On or about July 15, 2016 the employer did not ensure that an extension cord was inspected before each use and removed from service when deficiencies such as, but not limited to, missing ground pins and damaged insulation (sheath) were observed. An extension cord missing a ground pin exposed employees to hazards associated with electrical shock.
