29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved: On or about July 6, 2016, the employer exposed employees to electrical hazards (800VDC) while working on a power supply calibration system for the Keithley model 2260B-800-1 test stand. Other feasible safety related work practices were not implemented to ensure that the employee was aware of the potential exposure to live parts.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: On or about July 7, 2016, the employer exposed employees to electrical hazards (800VDC) by not ensuring that employees working on a power supply calibration system for the Keithley Model 2260B-800-1 test stand, were wearing heavy duty leather gloves.
