29 CFR 1910.23(a)(8)(ii): Every floor hole/opening into which persons could accidentally walk were not guarded by a floor hole cover of standard strength and construction: The employer is failing to ensure that employees are protected from a fall into a hole/floor opening a hazard when working in and around the strapping station of the brick packaging machine. This machine is located in the new plant at 3705 S. Coddington Ave. Lincoln, Nebraska. This hazard(s) was most recently observed in the following but not limited to areas of the brick packaging machine: a. On the left side of the strapping station a hole/floor opening that measured 12 1/2 inches wide by 36 inches long and was 72 inches from the next lower level exists. b. On the right side of the strapping station a hole/floor opening that measured 10 inches wide by 36 inches long and 72 inches from the next lower level exists.

29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer is failing to ensure that employees are not exposed to caught between hazards in the new Plant located a 3705 S. Coddington Ave. Lincoln Nebraska. This was most recently occurred on July 7 2016 at 0930 when an employee was seriously injured when reaching into the back side of the strap station in order to adjust the bricks within the jig and the machine unexpectedly moved causing the injuries to the employee.

This was most recently observed on July 14, 2016 outside the Old Plant at 3705 S. Coddington Ave. Lincoln, Nebraska. The Toyota forklift was in need of repair in that the foot control pedals were not equipped with slip resistant material or covers.
