29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. Steve's Bug Off Exterminating Company, Inc., 8046 Frankford Avenue, Philadelphia, Pennsylvania 19136-2616 - The employer did not ensure that the side rails of a metal extension ladder employees used to access the roof of a one-story building was extended at least 3 feet above the upper landing surface exposing employees to a fall hazard. Condition noted On or About 07/14/16.
