29 CFR 1904.1(a)(2): The company had more than ten (10) employees or more during the last calendar year, but did not keep OSHA injury and illness records and the establishment was not classified as a partially exempt industry under � 1904.2.    a) Silvana Trucking, Inc - The employer failed to keep OSHA injury and illness records for calendar year 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident     a) Silvana Trucking, Inc - An employee suffered fatal injuries from a work related incident that occurred on July 9, 2016.  It was not reported to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor until July 11, 2016.    No abatement certification or documentation required.
