29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: In the diaphragm assembly area of the facility the employer is failing to protect employees from an amputation hazard in that the two hand controls on the Richmond Engineering Diaphragm machine, model number R1A2101 failed to stop it cycle when the employee(s) take their hands off of the two hand controls. This continuation of the cycle allows an employee to place their hand in the pinch point between the diaphragm assembly and the crimper.
