29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: On or about July 15, 2016 the employer did not ensure that vertical rebar protruding approximately 4 feet above base level were equipped with protective caps. This condition exposed employees to lacerations or puncture wounds.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about July 15, 2016 the employer did not ensure that an extension cord was used as labeled. A "shop-made" extension cord attached to a metal receptacle box designed only for inside mounted use exposed employees to hazards associated with electrical shock.
