29 CFR 1926.502(f)(1)(iv): When the path to the point of access was not in use, a rope, wire, chain, or other barricade, equivalent in strength and height to the warning line, was not placed across the path at the point where the path intersected the warning line erected around the work area: a) On July 14, 2016, employees performing roofing work on a flat roof were exposed to a fall hazard of approximately 21.4 feet where a rope, wire, chain, or other barricade was not placed across the opening in the waste disposal area when it was not in use.
