29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:      (a)  Industrial Pipe Solutions Corp. at 54th Ave & Lincoln St., Denver, CO 80216:  On or before July 15, 2016 the employer did not conduct an evaluation of a storm-water manhole prior to entering the confined space, in that an employee climbed down the fixed ladder of a manhole to investigate a blockage of the storm water pipe.  The manhole was approximately 7 feet deep and had 3 to 4 inches of standing water at the bottom.  This condition potentially exposed the employee to a hazardous atmosphere.
