On or about July 15, 2016 two employees were repairing a sub panel on a 460 Volt Control panel and were burned by an Arc Flash created when contact with an employees screwdriver made contact with one lug of the three phase power wires energizing the control panel.

29 CFR 1910.333(b)(2)(iv): Verification of deenergized condition. The requirements of this paragraph shall be met before any circuits or equipment can be considered and worked as de-energized. a). On or about July 15, 2016 two employees working on a 480volt sub-electrical panel did not de-energize nor verify that the panel box was de-energized before the employees began work in the sub panel.

29 CFR 1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee did not use insulated tools or handling equipment when the tools or handling equipment might have made contact with such conductors or parts: a) On or about July 15, 2016 employees were making repairs to a 480 volt electrical service panel and did not use insulated tools or handling equipment exposing them to electrical shock and burn hazards. Two employees suffered electrical burns and arc flash injuries when one employee made contact with an energized wire with a non insulated screwdriver.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about July 15, 2016, employer did not ensure proper electrical protective equipment such as rubber gloves, rubber sleeves and eye protection, was worn by two employees who were completing electrical repairs to a sub panel electrical panel in the saw mill shop. Two employees were exposed to, and suffered Arc Flash burns to their face, neck and arms when one employee made contact with the 460volt electric wires, because they were not protected by any means of electrical personal protection.
