OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards: a) On or about 7/15/16 at the Canalside Ruins located on Lloyd Street, Buffalo, New York; employees cutting grass with Honda Walk Behind Lawn Mowers were operating the mowers on slopes (greater than 20 degrees) that were damp from morning dew and sprinklers. An employee slipped and fell and making contact with the mower blade and amputating two toes from his foot. ABATEMENT DOCUMENTATION REQUIRED

A workplace assessment to identify hazards and evaluate the need for, and type, of personal protective equipment required has not been completed. ABATEMENT CERTIFICATION REQUIRED
