29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. For the jobsite at 2441 Oak Myrtle Lane, Wesley Chapel, FL 33544 as observed on or about 07/19/2016, employees were exposed to fall hazards while working off of scaffolding that was not restrained from tipping by guying, tying, bracing, or equivalent means. The scaffolding was 24 ft. 8 in. high and only 5 ft. wide in locations on the north and south side of the site.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: For the jobsite at 2441 Oak Myrtle Lane, Wesley Chapel, FL 33544 as observed on or about 07/19/2016, employees were exposed to fall hazards while working off of scaffolding that was missing guardrails. a) For the north section of the scaffolding, employees were exposed to fall hazards of up to 20 feet off of the end of a side bracket. b) For the south section of the scaffolding, employees were exposed to fall hazards of up to 18 feet where the top-rail and mid-rail sections of guardrails systems were missing.
