On or about July 15, 2016, the Hitachi Crane, model #923261, Serial #7035312, all functional components of the crane had not been thoroughly inspected, an employee fatally injured when the crane cab separated from the super structure and fell to the ground below.

Prior to July 15, 2016, on the Hitachi Crane, Model #932261, Serial #7.35312, Employer identification #2, a monthly inspection noted that the boom cable wire needed lubrication which was not completed prior to the boom being lowered on or about July 15, 2016.
