29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Scrap Room, Building 99, 730 Worcester St, Springfield, MA On July 19, 2016 and at times prior, employees were exposed to hazards of potentially hazardous energy while engaged in the following activities covered by this section. a. Employees who threaded/fed sheets by hand into the scrap granulators without locking out the energy sources, were exposed to crushing and amputation hazards.
