Fixed stairways shall be maintained in safe condition and shall not be blocked:    a) On or about July 14, 2016, employees were exposed to a fall hazard on the wooden exterior stairs under the rail shed. Two stair treads were damaged and broken in half and missing wood.      b) On or about July 14, 2016, employees were exposed to a fall hazard on the wooden exterior stairs under the rail shed.  The east side stair support footing was damaged and missing large amount of wood.        c) On or about August 16, 2016, employees were exposed to a fall hazard at the metal step on the north side of a bridge platform on the conveyor catwalk in the chemical dry storage warehouse. The metal step was missing.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace, including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:    a) On or about July 15, 2016 in the dry chemical storage area, employees were exposed to diammonium sulfate, diammonium phosphate, urea, potassium chloride, and urea ammonium nitrate. These exposures occurred while mixing chemicals, bagging chemicals, and unloading the chemicals into the storage area by truck and barge. Due to the chemical reactivity with water and the amount of water on the floors and moisture in the air, there was a potential for air contaminant hazards, which included, but were not limited to ammonia to accumulate in the basement pit. No evaluation for chemical exposures had been performed by the employer.    The Frick Services, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.134(d)(1)(iii), which was contained in OSHA inspection number 992691, citation number 1, item number 1b and was affirmed as a final order on September 23, 2015 with respect to a workplace located at 800 Sun Drive, Portage, IN 46368.
