29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: a) 146 Demarest Avenue, Avenel, NJ 07001. The employer allowed his employees to work on and from the second floor interior landing when the guardrail system was not capable of withstanding 200 pounds of force, failed and the employee fell approximately 10 feet the first floor below. Occurred on or about 07/15/16.

The employer allowed his employees to traverse interior stairway from the first floor to the second floor when the stairway was not equipped with one handrail and one stair-rail system along each unprotected side or edge exposing the employees to fall hazards. Occurred on or about 07/15/16.
