29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:  South side of commercial building; Genie Aerial Lift (Model: S-65, Equipment Rental No:663183RA), On or about July 20, 2016, employees without fall protection were exposed to fall hazards, while reinforcing a flat roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  On the roof of a commercial building, South side, on or about July 20, 2016, employees without fall protection were exposed to fall hazards greater than 6 feet to the unpaved ground below, while reinforcing a flat roof.
