29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  a)  The loader on line 2 was exposed to noise levels at 67.02% of the action level dose (8-hour time weighted average sound level of 85 decibels) or an equivalent sound level of 87.1 dBA during 363 minute sampling period on August 3, 2016.  The employee could be exposed to noise induced hearing loss.  b)  The filler on line 2 was exposed to noise levels at 73.39% of the action level dose (8-hour time weighted average sound level of 85 decibels) or an equivalent sound level of 87.7 dBA during 359 minute sampling period on August 3, 2016.  The employee could be exposed to noise induced hearing loss.  c)  The filler on line 1 was exposed to noise levels at 72.85% of the action level dose (8-hour time weighted average sound level of 85 decibels) or an equivalent sound level of 87.7 dBA during 356 minute sampling period on August 3, 2016.  The employee could be exposed to noise induced hearing loss.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Throughout the establishment:  The employer did not provide proper training for temporary employees working with chemicals, such as,  but not limited to Spectracide Wasp & Hornet Killer, Evercide Intermediate 2756, UltraTec DP Concentrate 1.16-2.86, 0.5% Permethrin Lice Bed Spray and Simoniz Swell Grill & Oven Cleaner.
