29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 20 July 2016, and at times prior thereto, two employees were working within a trench where the surface was above their heads and a spoil pile at the edge of the trench without head protection, exposing the employees to the hazard of being struck by falling rocks, dirt, and debris.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY, MEASUREMENTS AS NEEDED, AND WHAT MEANS OF EGRESS, IF ANY, WAS PROVIDED)  On or about 20 July 2016, and at times prior thereto, two employees were working inside a trench to install new drainage lines where the trench was in excess of 4 feet deep without safe means of egress, exposing the employees to a fall hazard of approximately 7 feet 6 inches.

On or about 20 July 2016, and at times prior thereto, two employees were working within a trench that measured to be approximately 7 feet 6 inches in depth without a protective system, exposing the employees to the hazard of a cave-in and entrapment.
