29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. On 7/20/16, at the Johns Creek job site, employees were not using the prescribed Lockout procedures established for various equipment in the LS-3 production area. Employees were permitted to use interlock "jump keys" without procedures and machine interlocks as a means of lockout protection when providing service and maintenance to equipment.
