29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   On or about June 14, 2016, the employees were assigned the job task of replacing the seals in the hydraulic stacker and were not trained on energy control procedures for energy isolation. Employees were exposed to the hazard of being struck-by the unexpected gravitational energy from the out cylinder falling.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  On or about June 14, 2016, the employer failed to notify the Occupational Safety and Health Administration (OSHA) of an injury that required inpatient treatment and an amputation. OSHA became aware of the injury on July 12, 2016.
