29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a.) Select-Tron Industries, Inc., 1946 East 12th Steet, Erie, Pa, Eyewash in electroplating area, on or about July 20th , 2016, and times prior thereto: Employees worked with and around corrosive chemicals including, but not limited to, Muriatic and Sulfuric Acids during electroplating operations. The plumbed emergency eyewash station in the electroplating area was not in compliance with ANSI 358.1-2014 for Emergency Eyewash and Shower Equipment in that: 1.) The eyewash station was not operational, in that, upon activation, the eyewash did not provide flushing fluid to the employee's eyes and face during activation. 2) Weekly inspections of the equipment were not conducted. The emergency eyewash unit was not activated weekly to verify proper operation.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a.) Select-Tron Industries, Inc., 1946 East 12th Steet, Erie, Pa, Electroplating area/Muriatic acid tank area, on or about July 20th , 2016, and times prior thereto: Employees utilized a flexible (extension) cord to power a Coffing brand 500 pound hoist in the electroplating area of the facility. The flexible cord was used in lieu of the permanent wiring of the structure.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: a.) Select-Tron Industries, Inc., 1946 East 12th street, Erie, Pa, Electroplating area, on or about July 20th , 2016, and times prior thereto: Employees utilized immersion heaters to heat water and acids onsite. The conduit protecting the internal wiring of the Process Technology brand immersion heater (Model #QM2217-PQI) was disconnected/split exposing the internal wires of the heater.
