29 CFR 1926.703(a)(1): Formwork shall be designed, fabricated, erected, supported, braced and maintained so that it will be capable of supporting without failure all vertical and lateral loads that may reasonably be anticipated to be applied to the formwork. Formwork which is designed, fabricated, erected, supported, braced and maintained in conformance with the Appendix to this section will be deemed to meet the requirements of this paragraph. A. Graber and Graber Contractors Inc. worksite located at 9715 Rosedale Rd., Hicksville, Ohio: On or about July 12, 2016, the employer did not ensure the formwork on which a concrete deck was being poured was inspected prior to conducting the pouring operation. A portion of the formwork collapsed resulting in an employee falling approximately 12.00 feet sustaining a serious leg injury.
