29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means:  a) 1609 Federal St, Philadelphia, PA - Employees working on a fabricated frame scaffold approximately 39 feet high, 28 feet long and 5 feet wide with a height to base ratio exceeding four to one did not have the scaffold guyed, tied or braced to the structure thus exposing the employees to scaffold collapse hazard. This condition was observed on or about July 18, 2016.

29 CFR 1926.451(f)(12): Wind screens shall not be used unless the scaffold is secured against the anticipated wind forces:  a) 1609 Federal St, Philadelphia, PA - Employees working on a fabricated frame scaffold approximately 39 feet high, 28 feet long and 5 feet wide which was not secured to the structure and covered with tarpaulins thus exposing the employees to scaffold collapse hazards. This condition was observed on or about July 18, 2016.
