29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  a) For the jobsite at 5630 Golf Pointe Dr., Sarasota, FL 34243, as occurring on or about 07/14/2016, employees were exposed to fall hazards while working off of ladders at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-half of the working length of the ladder. On 07/14/2016, an employee fell from a 24 foot extension ladder onto concrete and was hospitalized.

29 CFR 1926.1053(b)(17): Fixed ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, or corroded components, were not withdrawn from service until repaired:  a) For the jobsite at 5630 Golf Pointe Dr., Sarasota, FL 34243, as occurring on 07/14/2016, a 24 foot extension ladder was missing rubber grip on one of its feet and was still in service. On 07/14/2016 the ladder slipped and the employee fell off of the ladder onto concrete and was hospitalized.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  a) For the jobsite at 5630 Golf Pointe Dr., Sarasota, FL 34243, as occurring on 07/14/2016, employees working off of ladders had not been trained on ladder safety. On 07/14/2016 an employee fell off of a 24 foot ladder and was hospitalized.
