29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    The employer is failing to protect employees from struck by and crushing hazards. This was most recently documented on or about July 18, 2016 at a jobsite located at 30979 Highway 60 in Pierce City, Missouri. The employer failed to ensure exposed employees were protected from struck by and crushing hazards while working in the debarking and log haul area as employees move and rotate logs using cane hooks around the log haul and 2nd log deck, go under the 2nd log deck to attach a chain to logs to haul them out as well as to clean under the 2nd log deck.

This was most recently documented on or about July 18, 2016 at a jobsite located at 30979 Highway 60 in Pierce City, Missouri. The employer failed to ensure exposed employees were protected from potentially hazardous energy in that machine specific lockout procedures did not include specific procedural steps for shutting down and securing machines, nor were specific procedural steps for the placement, removal and transfer of lockout devices included in the company procedures.
