Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer must report the in-patient hospitalization, amputation, or loss of an eye to OSHA:    (a) Facility, 22 Wharf Street, Dunlevy, PA:  On or about July 18, 2016 the employer reported an incident to the Pittsburgh OSHA office for an injury that occurred on  July 14, 2016.  An employee injury that resulted in an in-patient hospitalization was not reported to OSHA within 24 hours of the employer knowing the incident occurred.
