29 CFR 1904.39(a)(2): 29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours. a) Mare Solutions Inc. / 135 East 38th Street, Erie, PA: On or about July 21, 2016 and times prior thereto, the company did not report a work related hospitalization within 24 hours of it occurring. The hospitalization occurred on or about June 23, 2016 at approximately 11:50 AM. The hospitalization was not reported to OSHA until July 14, 2016 at about 4:00 PM.
