29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) 720 Museum Road - Employees working on a 14-12 pitch slate roof with unprotected sides was not protected from falling approximately 12 feet to the ground below, thereby exposing employees to fall hazards on or about June 30, 2016. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) 720 Museum Road - The employer did not ensure that employees exposed to fall hazards were trained on how to properly wear, operate and maintain a personal fall arrest system, thereby exposing employees to fall hazards on or about June 30, 2016.
