(a) South Charleston: On or about July 22, 2016 employees were carrying material including, but not limited to, bundles of shingles without the use of foot protection to prevent injury from physical contact.

(a) South Charleston, Roof: On or about July 22, 2016 an employee was using a Bostitch pneumatic nail gun to install shingles on a residential roof without the use of eye protection.  (b) South Charleston, Roof: On or about July 22, 2016 an employee was wearing sunglasses without protective lenses in accordance with the Z87.1-1968 requirements, while working in close proximity to nail gun operations.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) South Charleston, Residential Roof: On or about July 22, 2016 and times prior, employees were removing old roofing materials and replacing with new on a 5:12 pitched roof that measured approximately 10 feet from the eave to the ground and did not have any fall protection systems in place.
