29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     On or about July 22, 2016, at the construction site where employees were exposed to a fall hazard while using an extension ladder to access a roof without extending the ladders side rails at least three feet above the roof landing or securing the top of the ladder to the roof landing or other rigid support.

SAMUEL VENCES DBA STONE GATE ROOFING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(11), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 534558, CITATION NUMBER 1 ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 06, 2012 WITH A FINAL ABATEMENT DATE OF NOVEMBER 28, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 24900 E. SAM HOUSTON PKWY SOUTH, PASADENA, TX 77502.
