Clearwater Construction, Inc. - On or about 7/21/16, and at times prior thereto, a light was being used in the gatehouse of the dam to provide illumination. The light was not designed to be used in a construction environment. The light bulb was not protected from damage and the light was not equipped with a heavy duty cord.

29 CFR 1926.1419(f): During operations requiring signals, the ability to transmit signals between the operator and signal person were not maintained. When that ability was interrupted, the operator did not safely stop operations requiring signals until it is reestablished and a proper signal is given and understood. A.) Clearwater Construction, Inc.; Pymatuning Dam - On or about 7/21/16, an employee was struck by a wooden form that was being lifted by a crane. The employees was standing in the area of the lift and was struck by the load. The person at the top of the opening was attempting to guide the load and stay in contact by radio with the crane operator.
