29 CFR 1910.178(m)(3): Unauthorized personnel were permitted to ride on industrial truck(s)  On or about July 5, 2016:  a) An employee was allowed to ride on the forks of a powered industrial truck, to stabilize a load. The employee fell off of the forks during travel and being struck by the forklift, resulting in multiple fractures to the riders foot.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
