29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)135 Intervale Rd., Fitchburg, MA: On or about July 18, 2016, the employer had not developed or utilized a procedure for the control of potentially hazardous energy from the trim/cold press power press to ensure employees were protected from the unintended energization during repair and maintenance activities covered by this section.     Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1904.39(a)(3): The employer failed to report the fatality, in-patient hospitalization, amputation, and/or loss of an eye using one of the following methods: (i) By telephone or in person to the OSHA Area Office that is nearest to the site of the incident. (ii) By telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742). (iii) By electronic submission using the reporting application located on OSHA's public website at www.osha.gov:  a)135 Intervale Rd., Fitchburg, MA: On or about July 18, 2016 the employer made their initial report of the amputation that occurred on July 16, 2016 at this facility, which was more than twenty-four (24) hours after the employee suffered the amputation.
