29 CFR 1926.453(b)(2)(v): A body belt and lanyard was worn but was not attached to the boom or basket when working from an aerial lift.  On or about July 22, 2016 at the location of the new LA Fitness under construction,  two employees were working in the basket of an aerial lift installing and finishing stucco work.  One of the employees was wearing their fall protection an attached to the anchor point in the basket of the aerial lift the other was wearing their fall protection, but not attached to the anchor point installed in the basket  of the aerial lift.  The employee was exposed to a fall of thirty (30) feet to the lower level when not attached to the anchor point in the basket of the aerial lift.
