29 CFR 1910.107(b)(5)(i): The spraying operations except electrostatic spraying operations shall be so designed, installed and maintained that the average air velocity over the open face of the booth (or booth cross section during spraying operations) shall be not less than 100 linear feet per minute. Electrostatic spraying operations may be conducted with an air velocity over the open face of the booth of not less than 60 linear feet per minute, or more, depending on the volume of the finishing material being applied and its flammability and explosion characteristics. Visible gauges or audible alarm or pressure activated devices shall be installed to indicate or insure that the required air velocity is maintained. Filter rolls shall be inspected to insure proper replacement of filter media.  On or about July 14, 2016; at the facility: The spray booth did not have visible gauges or alarms to ensure that at least 100 LFM or air flowed through the booth during spray painting operations.

29 CFR 1910.107(f)(1): Conformance. In sprinklered buildings, the automatic sprinkler system in rooms containing spray finishing operations shall conform to the requirements of 1910.159. In unsprinklered buildings where sprinklers are installed only to protect spraying areas, the installation shall conform to such standards insofar as they are applicable. Sprinkler heads shall be located so as to provide water distribution throughout the entire booth.  On or about July 14, 2016; at the facility: The spray booth was not protected from fire by a sprinkler system. The employer uses flammable chemicals such as Acetone in daily operations. This condition potentially exposes employees to burns in the event of fire.

Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On or about July 14, 2016; at the facility: Employee was observed operating a forklift (Toyota, Model 8FGU25, SN 12113) without using the seat belt, which was installed on the vehicle. This hazard exposes the employees to accidents associated with falling out of the powered industrial truck.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  On or about July 14, 2016; in the shop area, near the loading dock doors: Guarding was not maintained for a box fan with 2" X 2" openings. Employees potentially exposed to finger injuries.
