29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:      A.)  M & C Complete Construction LLC; 16 Cedar Street, Warren, PA - On or about 7/20/16, and at times prior thereto, employees were not wearing safety glasses while performing roofing operations.  The employees were performing work activities that included, but were not limited to, utilizing nail guns to attach shingles.

M & C Complete Construction LLC; 16 Cedar Street, Warren, PA - On or about 7/20/16, employees had not received training from the company on ladder safety and use.  The employees were utilizing ladders to access a residential roof to performing roofing activities that included, but were not limited to, installing a lay over roof.
