On or about July 25, 2016 and at times prior, employees were using a body belt and a non-shock absorbing lanyard as a personal fall arrest system while operating a Crown stockpicker forklift; model SP 3400, serial 1A295585. The equipment did not meet the mandatory personal fall arrest system requirements per 1910.66 Appendix C.
