29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a) Jobsite:  Employees working on the ground level were not protected from being struck-by falling objects and debris associated with the stripping of old roof materials where protective helmets were not worn.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) Jobsite:  Employees were engaged in the removal and replacement of a flat roof without the use of personal fall arrest systems, warning lines, safety monitor, or guard rails exposing them to a fall of approximately thirty-four (34) feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Jobsite:  An aluminum extension ladder placed on the third floor balcony that was used to access the roof did not extend three feet above the landing exposing employees to a fall of approximately eight (8) feet to the balcony deck.
