(a) Date of Violation: On or about 05-29-2016 and times prior thereto, located at the Virginia International Gateway Marine Terminals, Berth #2, 1000 Virginia International Gateway Blvd. Portsmouth, VA 23703, Pier-side, an employee was exposed to a left leg amputation from a struck-by hazard, where two straddle carrier shuttle-trucks collided resulting in one tipping over because the employer did not direct employees to slow down at crossaisles or other locations where visibility is obstructed.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
