A) Container Berth Area #1, vessel St. Louis Express, on or about June 17, 2017, there were no marked or designated areas for pedestrians to utilize when crossing vehicle traffic lanes, exposing employees to a struck-by hazard.

29 CFR 1918.85(k)(2): When fall protection systems required by 29 CFR 1918.85(j) were employed, fall protection systems that were subjected to impact loading were used before being inspected and determined by a designated person to be undamaged and suitable for use.    A) Shore side of vessel Spirit of Tokyo, on or about June 22, 2017, the employer had not ensured that each fall protection system subject to impact loading be withdrawn from service and not used again until inspection and determined by a designated person to be undamaged and suitable for use.
