29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    Side of building:  A pump-jack scaffold system with four (4) poles was erected approximately forty (40) feet tall where the base of the poles were resting on the ground and a non-flat rock.  This exposed the employees to fall and struck-by hazards from shifting or collapsing scaffolding.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.    Side of the house:  A pole for a pump-jack scaffold system had been erected approximately eight (8) inches from the live home electrical service line exposing employees to electrocution and fall hazards.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    Side of house:  A pump-jack scaffold system had been erected with poles approximately forty (40) feet tall without bottom or mid bracing, without mud sills, and without being plumb.  The erection was not supervised by a competent person exposing employees to fall and struck-by hazards from collapsing scaffolding.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    Side of building:  Poles used for pump-jack scaffolding approximately forty (40) feet tall were not secured to the structure at the bottom or other points as required allowing the employees to move the bottom of the poles close to the building exposing them to fall and struck-by hazards.
