29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: (a) On or about July 22, 2016, V & C General Construction, LLC., did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. Workers were observed using a ladder to access the roof of a residential building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) On or about July 22, 2016, V & C General Construction, LLC. did not ensure that its employees were protected from falls while working at heights greater than 6 feet. Employees of V & C General Construction, LLC. were exposed to fall hazards of up to 17 feet while engaged in roofing activities on a residential building with no fall protection in place. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). V & C General Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1060478, citation number 1, item number 1 and was affirmed as a final order on July 2, 2015, with respect to a workplace located at 10422 E. Prentice Ave. Greenwood Village, CO 80111.
